Redwoods Rural Health Center is providing notice of a data security incident involving TriZetto, a third-party vendor that supports our electronic medical record system through OCHIN. TriZetto experienced unauthorized access to its systems and notified us on December 15, 2025, that certain patient information may have been exposed. The information involved may include name, date of birth, contact information, Social Security number, and certain health or insurance information; not all patients were affected, and there is currently no evidence of misuse. TriZetto secured its systems promptly, and we are working closely with OCHIN to reinforce safeguards and review our own processes. Affected individuals will receive direct written notification from TriZetto. This website notice is being posted to ensure awareness for individuals whose contact information may be outdated or undeliverable. Patients are encouraged to remain alert for suspicious activity, such as unexpected bills or requests for personal information, and to contact their insurer or financial institution if concerns arise. TriZetto has established a dedicated call center at (844) 572-2724 (available beginning January 5, 2026), and additional information—including Spanish-language resources. We regret any concern this may cause and appreciate the trust our patients place in our care.
