Full-Time Integrated
Behavioral Health Clinician ​
Redwoods Rural Health Center is implementing the Primary Care/Behavioral Health (PC/BH) model within the organization and works with medical clinics and programs to instill integrated care coordination into the culture of the organization.

Provides development and evaluative assistance and general oversight to the program.

Provides clinical supervision of and instruction to Integrated Behavioral Health Clinicians and graduate level interns as part of an organization Workforce Development Program.

Provides direct patient care both within the PCBH model and/or long-term therapy for patient/ population needs.

Education: Doctorate (PsyD or PhD) and/or master’s in social work, Marriage and Family Therapy, or related field in Psychology

Experience: Five years working with people with mental or behavioral health needs and clinical supervision experience. Two years of leadership experience is required. Experience in the medical field is preferred. Will consider associate level clinicians (ACSW, AMFT, similar), IF they have already achieved 2000 hours of clinical supervision towards their clinical license.

Licenses/Certificates/Registration: Either fully licensed or associate level clinicians will be considered for this position. Full-Time Physician Assistant​
Redwoods Rural Health Center (RRHC) is seeking a dedicated and compassionate Physician Assistant to join our integrated health care team. The Physician Assistant will work under the direct supervision of the Medical Director, providing comprehensive primary medical services. Responsibilities include diagnosing and treating patients of all ages, coordinating with internal and external providers, ordering and interpreting diagnostics, prescribing medications, and contributing to ongoing quality improvement and community outreach efforts.
Ideal candidates will hold a valid, unrestricted California PA license and DEA registration, be board certified or eligible, and have current ACLS/CPR certification. Successful applicants will bring a deep understanding of modern medical practice in a community health setting and demonstrate strong communication skills, cultural sensitivity, and a commitment to RRHC’s mission. Experience in a family practice or outpatient clinic setting is preferred. Redwoods Rural Health Center is seeking a LMFT/LCSW therapist who can work anywhere in the country if licensed in California to provide therapy to clients referred by health providers or self-referred in individual and group sessions.
JOB DUTIES & RESPONSIBILITIES:

Carry out program enrollment, intake and care management

Assist with quality improvement, chemical dependency programs, and the integrated behavioral health program

Support and carry out crisis assessment as needed to assess a patient’s danger to self, danger to others and/or grave disability

Serve as a consultant to primary care staff on mental health and behavioral issues

Full-Time Registered Nurse​
​​
Redwoods Rural Health Center is seeking two Registered Nurses for our Redway location. The Clinical (RN) provides general nursing care to patients in the clinic and over the phone. The RN plays an essential role in providing patient access through visits using protocols and transitioning to provider whenever able or needed. Within the Care Team, the RN works as a point person for daily triage.​​
​
RN RESPONSIBILITIES:

Responsible for phone triage assessment and/or in-person triage for patients with urgent needs and schedules patients after appropriate triage.

Actively supports provider workflows by managing and addressing items in provider inboxes, such as PAQs (Provider Action Queues), MyChart messages, in accordance with nursing scope of practice and clinic protocols.

Responsible for limited types of patient visits including immunizations, education, and co-visits as determined by the RN Clinic Manager or Medical Director.

Explains procedures and treatments to patients to gain cooperation, understanding, and allay apprehension.
-
Responsible for patient assessment based on chief complaint and treatment as outlined by Nursing Protocols or provider direction.

Observes patient, records, significant conditions, and reactions, and notifies supervisor or Provider of patient's condition and reaction to drugs, treatments, and significant incidents.

Communicates directly with clinic providers regarding patient assessment and needs; transitions patient to the provider as needed.

Utilizes Situation, Background, Assessment, Recommendation (SBAR) warm hand-off method.

Recommends and/or administers medications and vaccines as ordered by medical provider or by protocol.

Responsible for coding each visit accurately.

Accurately records patient history, exam notes, medication history, ongoing care, and referrals in employee medical record (EMR) according to RRHC Protocols.

Manages inbox and appropriate tasks in their assigned providers’ inboxes; ensures team inbox tasks are completed in a timely manner according to team role and scope.

Assists in Chronic Care management, such as outreach calls, care coordination, etc.

Assists and/or facilitates routine daily huddles within the team.

Provides oversite to the MA working in the Care Team as needed for safe vaccine delivery.

Maintains awareness of the comfort and safety needs of the patient.

Responds to medically urgent situations based upon nursing standards, policies, procedures, and protocol.

Attend and participate in meetings and Quality Improvement activities as required.

Identifies problems related to patient services and makes recommendations for improvement.

Responsible for infection prevention program and efforts within the clinic and serving as the infection prevention contact for the County.

Under Medical Director, manages the on-site pharmacy and ensures security, documentation of medications that are brought in (samples, donated, purchased or through), schedules the quarterly Pharmacy Audit / Inspection carried out by a licensed contracted Pharmacist. Oversight of Pharmacy relations and prescription refills done by Medical Assistants for providers.

Partners with providers in a fast-paced, team-based care environment to improve timely care delivery, close care gaps, and coordinate services.

Maybe asked to manage medication refills per approved RRHC protocol and within the RN scope of practice, ensuring timely and accurate refills for patients.

Full-Time Medical Eligibility Assistor​

Redwoods Rural Health Center is hiring an in-person Eligibility Assistor for our Redway location to help uninsured individuals and low-income families gain access to health insurance programs. In this role, you’ll guide patients through applications for Medi-Cal, sliding scale, and other supportive services, while providing clear, culturally sensitive information. You’ll also verify insurance for patients before appointments, maintain accurate records, and ensure all work is handled with the highest level of confidentiality.
RESPONSIBILITIES AND DUTIES:

Conducts insurance verification for the Medical department prior to patient appointments.

Notifies the Supervisor and Front Desk of patient insurance needs/follow up.

Reaches out to uninsured patients prior to appointment to offer sliding scale application or Medi-Cal application.

Assists applicants in properly completing and submitting applications with correct income verification documents.

Answers questions pertaining to the application process.

Provide comprehensive and culturally sensitive information about public insurance programs and benefits. .

Refers applicants to other needed social services as necessary.
