Partnership Healthplan Notice of Data Breach
You maybe receiving a notice from Partnership HealthPlan of California (“PHC”) to make you aware of an incident that may affect the security of some of your information. Below is some information from PHC.
WHAT HAPPENED? We were informed previously by PHC of a data security incident, which they have worked to resolve.
WHAT DID PHC DO WHEN THE INCIDENT WAS DISCOVERED? PHC launched a detailed investigation into the nature and scope of the incident with the assistance ofthird-party forensic investigators. PHC also took steps to secure its systems and information.
WHY DID I GET THIS LETTER/NOTIFICATION FROM THEM? PHC is in the process of notifying all potentially impacted individuals of the incident. The notification includes important information for those individuals.
