Patient Instructions for
Medical Record Requests

Redwoods Rural Health Center has partnered with HealthMark Group to ensure the accurate and timely completion of medical record requests.

 

How?

Requests may be submitted electronically to HealthMark’s Request Manager at https://requestmanager.healthmark-group.com. Once logged in, select “Submit Request” from the menu options and enter all required fields to provide an authorization directly to HealthMark. Your medical record request will be processed, and a notification will be sent via mail or

email once complete and available for download.

Additional Ways to Request Your Medical Records:

Request your Medical Records through your MyChart

Once logged into your MyChart account, select Your Menu in the top left corner. From the drop-down menu navigate to the My Record section, select Document Center. A new page will open Document Center, from here select the appropriate option for your request.

 

Request your Medical Records in person at either the Redway Clinic & Fortuna Clinic

Our front desk staff will provide you with a Medical Records Release Request form to fill out in the office.

 

 

Download the Medical Records Release Form from RRHC.org

Located under the Download Forms section. Fill this out and return to our Redway Clinic or Fortuna Clinic.
